HEART ATTACK: Salman’s story

2nd October 2023Fitness, Medical, Patient information, Self care

Dear Patients,

This is Dr Salman Uddin one of the partners at the surgery and I have been working at this organisation since 2017. Some of you may know that I had a heart attack back in July 2020. This was a difficult experience, but I feel like it is also an opportunity for me to share my story in the hope that we can raise awareness of heart related conditions. I have been volunteering with the British Heart Foundation (BHF), and I was involved in producing a video for them. In addition they have also interviewed me, about my heart attack story, for a podcast called Ticker Tapes.

Please feel free to watch the video and listen to the podcast if you can.

Repeat prescription requests

13th October 2021Patient information

From October 1st 2021 we will no longer be accepting prescription requests made by telephone.

In the interest of safety and efficiency, we kindly request that repeat prescription requests are made downloading and using the NHS App, e-consult on our website, email (thccg.citysquare1@nhs.net) or on Patient Access (pin required).  These services mean you can quickly and conveniently order your repeat prescriptions and your request will come straight through to our clinical system. There is also a box to send any comments or questions relating to your request and the option to change your nominated pharmacy.

If you need any help with how to order your prescriptions, please ask the reception team. For patients who do not have internet access, we will continue to accept paper requests via the box at reception and orders made through local pharmacies.
