A Clinical Pharmacist (CP) is a healthcare professional who specialises in optimizing medication use to improve patient outcomes. They collaborate with physicians, nurses, and other healthcare providers to develop treatment plans, ensure safe and effective medication therapy, monitor patient progress, and provide education on drug interactions, side effects, and proper usage. Their role is critical in managing chronic diseases, reviewing prescriptions, and contributing to patient safety and care.

Nurse Practitioner

Advance Nurse Practitioners are experienced Registered Nurses who have undertaken additional academic training at Master's level.  Acting within their professional boundaries provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.

A First Contact Physiotherapist (FCP) is a highly trained clinician who serves as the first point of contact for patients with musculoskeletal (MSK) issues. Instead of seeing a GP, patients can directly consult an FCP for assessment, diagnosis, and treatment of MSK conditions such as back pain, joint problems, or sports injuries. FCPs can order diagnostic tests, prescribe medications, and refer patients to specialists if needed.

You may be eligible for an appointment with a physiotherapist for the following problems:

  • All soft tissue injuries, sprains, strains or sports injuries
  • Arthritis – any joint
  • Possible problems with muscles, ligaments, tendons or bone, eg tennis elbow, carpal tunnel syndrome, ankle sprains
  • Spinal pain including lower back pain, mid-back pain and neck pain
  • Spinal-related pain in arms or legs, including nerve symptoms, eg pins and needles or numbness
  • Changes to walking
  • Post-orthopaedic surgery

Data Lead

Data lead ensures that surgery meets NHS and Local authority KPIs and maintains high-quality data on the clinical system. They also provide in-house training where possible and help improve the systems in use or develop new ones. They will help develop strategies and implement plans for different patient cohorts to ensure patient care is delivered.

Communicating and liaising with services outside CMSG to help improve services that the surgery provides and submitting audits to authorities.

Primary Care Co-Ordinator

Care coordinators review patients’ needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribers and other professionals where appropriate.

They work with people around the patient to improve their understanding of the patient's conditions and support needs. They work collaboratively with the GPs and other clinicians at the CSMG to improve support and outcomes of the patient’s medical needs.

They ensure that our long-term conditions patients have a patient centric journey through CSMG, with less repeated appointments and more satisfactory care and information.

They join integrated care meetings and contribute information regarding patients and services required for vulnerable patients at CMSG. They identify and act on any additional support required for patients highlighting any safety concerns to their main clinical lead or related clinicians.

The receptionists are your first point of contact with our practice and will try to meet your requirements. They answer the phone, make appointments and give general non-clinical advice. The admin team are responsible for nonclinical  patient welfare such as insurance forms and medical requests.  They also deal with any IT problems, premises management and prescription management.

Attached Staff

This practice allows community health service clinical staff directly involved in your care to access parts of your medical record. Your consent will be asked before this can take place. If you do not wish this to happen, please let the practice know.
